Monday, 8-13-18, between 12:30-1:30pm
Bowl w/ white rice, black beans, barb, sr crm, cheese, chips, red and green salsa.
Had food poisoning about 4 hours later. It was my first meal of the day as I have coffee for breakfast. That I make at home.

I ordered a burrito bowl with white rice, chicken, tomato salsa, cheese & sour cream. I was feeling nauseas within hours of eating accompanied by headache, stomach cramps & diarrhea.
The purchase date was July 26th.  | Symptoms: Diarrhea, Nausea, Cramps, Headache, Stomach Pain
