- comments: observed food buildup and debris on interior and exterior surfaces of prep tables, prep coolers, walk-in cooler, holding units, shelving units, and cooking equipment. Instructed manager to clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: observed leaking faucet on 3-compartment sink. Instructed manager to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in unisex washrooms. Instructed manager to provide.

55. Physical facilities installed, maintained & clean
- comments: observed damaged floor tiles and baseboards in walk-in cooler, walk-in freezer, prep, dish washing and utility areas. Instructed manager to replace and maintain. Observed missing grout on floor tiles in prep, storage, and dish washing areas. Instructed manager to repair and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed dust buildup on interior surfaces of walk-in cooler, upper exterior surfaces of ice machine, light fixtures, and ceiling vents, food spilss and debris on walls behind soda dispenser unit and debris in light shield behind front counter. Instructed manager to clean maintain.

- comments: observed loose light shield in front prep area and above breading in rear storage area. Instructed manager to repair.
