- comments: must label food storage containers when food is not in original package.

- comments: observed evidence of 6 rat droppings on floor next to food storage container. 1- live roach crawling on wall behind glass pop cooler. Appx. 7-10 live small flies on wall near grease trap and in toilet room. Must remove rat droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/2/22. (priority foundation 7-38-020(a)(citation issued)

- comments: observed an appx. '1/2-3/4' gap along bottom of front door. Must make door tight fitting.

- comments: must repair or replace missing door handle on glass reach in cooler,worn door gaskets on 1- door glass reach in cooler,3- door prep cooler. Must remove cardboard from lower shelf of table. Must remove rust from lid of chest freezer.
