- comments: pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty nor certificate posted to view while tcs ready to eat foods are being prepared,handled and served such as ground beef,italian beef,steak,sliced tomatoes,etc) must provide , post and be on site. (priority foundation 7-38-012)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand drying device at hand sinks in prep area and toilet room. Pic sent a customer to the store during inspection to purchase paper towels. Must provide and maintain at all times. (priority foundation 7-38-030(c)(cos)

- comments: must provide thermometers for both glass reach in coolers.

- comments: must label food storage containers when food is not in original package.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 25 or more rat droppings on floor behind glass reach in cooler and next to chest freezer in rear dish washing area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 3/13/20. (priority foundation 7-38-020(a)

- comments: must store plastic wear with handles in upright position.

- comments: must remove rust from lower shelf at prep table.
