16. Food-contact surfaces: cleaned & sanitized
- comments: noted the chlorine chemical dish machine not dispensing sanitizer while pots/pans being washed. 0 ppm chlorine noted after several attempts. Machine tagged held for inspection and must not use until repaired, properly dispenses sanitizer and c. D. P. H notified for re-inspection and tag removal. Priority violation 7-38-025 citation issued.
38. Insects, rodents, & animals not present
- comments: noted rat droppings in the following areas throughout the premises: approximately 50 droppings behind the kitchen tall cooler and deep fryer, 10-15 behind the kitchen box freezer, 10- 15 under the kitchen three compartment sink and dish machine. Noted 5-10 droppings under the stage in main 1st floor area. Approximately 15 to 20 droppings in the basement along baseboards throughout. Recommend having the pest control co. Service establishment for rats, must remove all droppings, sanitize all affected areas and maintain. Priority foundation violation 7-38-020(a) citation issued.

- comments: noted a small pan inside the bulk flour container being used as a scoop. Must provide scoops with handles for all bulk foods to be stored inverted the the food items. ----ice scoop noted stored on top of the basement ice machine. Must store ice scoops in a clean container.

- comments: clean aprons must not be stored on the floor of the basement.

- comments: must not use plastic grocery bags as food containers inside the tall reach-in freezer.

- comments: exterior of all cooks line equipment with excessive grease. Must clean and maintain.

- comments: dish/prep area hand washing sink unable to complete turn water off with a steady slow trickle/leak of water. Must repair.

- comments: floors in the following areas with dirt,grease and debris: throughout the kitchen under all equipment, dish area, throughout the basement, throughout the basement walk-in cooler. Must clean and maintain all. ----wall at the cooks line grill with excessive grease. Must clean and maintain. Wall next to the kitchen hot water tank with cotton like insulation. Must remove to make wall smooth and cleanable. ----water damaged ceiling tile in the 2nd flr. Bar women's washroom. Must replace.

- comments: no proof of food handler training or certificates for all required employees. Must provide.

- comments: previous core violations from 8-10-23 report #2579931 not corrected. #38- noted front entrance doors to the building not rodent proof with gap at the bottom of door (about 1/4 inch). Instructed to seal gaps to prevent entry points for pests. #55- instructed to clean and maintain floor drains behind the bar of the 1st floor. Priority foundation violation 7-42-090 citation issued.
