- comments: 2-101. 11 observed the person in charge (at the beginning of the inspection) without a city of chicago food service sanitation manager's certficate. Instructed all persons in charge must obtain
a city of chicago food service sanitation manager's certificate and be on site at all times the facility is open and operating. Priority foundation. Citation issued under violation #2.
2. City of chicago food service sanitation certificate
- comments: 2-101. 11 observed no city of chicago food service sanitation manager on site while facility is handling, prepping and serving tcs foods (deli meat) to the public. Instructed a city of chicago food service sanitation manager must be on site at all times. Priority foundation. Citation issued 7-38-012.
8. Hands clean & properly washed
- comments: 2-301. 14 observed a food handler collecting payments (money) and then proceed to put on gloves to handle food without washing hands in between tasks. Instructed to always wash hands after each task and never put on gloves without washing hands to handle any foods directly after handling money. Priority violation. Citation issued 7-38-010.

- comments: 6-301. 12 observed no hand drying provision at the hand washing sink located next to the 4 compartment sink in the rear dish washing area. Instructed to provide a hand drying provision such as paper towels at all hand washing sinks at all times. Priority foundation. Citation issued 7-38-030(c).

- comments: 4-602. 11(a) observed a black substance on the ice machine panel directly touching ice that is for human consumption. Instructed to clean and sanitize. Priority violation. Citation issued 7-38-005.

- comments: 4-602. 11(b-e) observed a stained cutting board with deep dark grooves attached to the prep coolers at the front food prep area. Instructed to remove stains or replace cutting boards.

- comments: 2-402. 11 observed a food handler wearing a hair restraint without hair properly restrained. Instructed to always properly restrain hair at all times.

- comments: 4-601. 11(c) observed walls with accumulated dirt debris and standing water under the 4 compartment sink. Instructed to remove dirt debris on the walls and never allow water to accumulated and remain on the floor under the 4 compartment sink.

- comments: 5-204. 12 unable to locate the back flow prevention device on the ice machine. Instructed to locate.

- comments: 5-402. 13 observed the front food prep area hand washing sink and the rear 4 compartment sink not properly draining with wastewater/sewage backing up into the compartment of said hand washing sink and wastewater/sewage backing up in 3 out of 4 compartments of the 4 compartment sink. Instructed to repair and never allow wastewater/sewage back up at any sinks at any time. Priority violation. Citation issued 7-38-030(c).

- comments: 6-501. 12 observed accumulated dirt debris on the floor throughout the entire food prep areas, under the 4 compartment sink in the rear, under the prep coolers in the front food prep area and under all storage shelves. Instructed to clean all.
