- comments: no written employee health policy on site. Must provide for all employees. Priority foundation violation 7-38-010 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: no written cleaning procedure or required equipment for a vomiting/diarrhea event. Must provide. Priority foundation violation 7-38-005 citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: menu does not disclose and inform consumers the specific menu items that are raw or under cooked and a potential hazard of consuming such food. Must provide a consumer advisory that discloses and reminds customers of such items. Priority foundation violation. No citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 75 to 100 mice droppings throughout the facility, on shelving throughout cooks line, on storage shelving of pizza boxes, on floor under metal shelving, on cooks line window sill, behind equipment, along baseboard in the employee washroom and basement. Instructed to remove all droppings, clean and sanitize all effected areas. Also noted approximately 50 small drain flies throughout the bar area under sinks, in sink drains, and flying throughout the area. Instructed to eliminate all fly activity. Recommend having pest control co. Service establishment for mice and flies. Priority foundation violation 7-38-020 (a). Citation issued.

- comments: large 3 ft x 3 ft open grate with 1 inch gaps to the outdoor rear of the building in the ceiling of the basement where foods stored. Must completely seal. All outer openings must be sealed, tight fitting, insect and rodent proofed.

- comments: must not store clean dishes for service in the outdoor dining area. All table wares must be stored inside the premises. -----basement ice scoop must be stored in a clean container.

- comments: all clean stored pots, pans throughout must be stored inverted.

- comments: must not use tape or plastic as a means of repair or padding on the corner of the metal prep table or as handles for the cooler and freezer doors. ----must not use porous tiles as a means of elevation for the metal legs of the metal rack storage shelving throughout the basement. Surfaces must be smooth and cleanable.

- comments: extremely soiled/blackened plastic dish racks. Must clean or replace. ----metal door of the walk-in freezer in poor repair with holes, open gaps in seams, bent metal and exposed insulation. Must repair to be smooth and cleanable or replace. Wooden pizza paddles in extremely poor repair with splintering wood along all sides. Must replace.

- comments: leak from piping onto the floor from the bar dish washing machine. ----extremely slow drain on the kitchen prep hand washing sink. ----leak at the nozzle on the utility service sink. Must repair all.

- comments: floor and floor drain under the kitchen dish washing machine, floors under the cooks with dirt, grease and debris. Must clean and maintain all.

Ate arugula that came in a grain and chickpea salad meal kit. Couldn’t sleep later that evening because of nausea. Repeated trips to the bathroom because of diarrhea. To date, this company has not received reports of illnesses related to the consumption of this product; however, the recall from the market began after the FDA found it to contain Yellow Oleander.

For easier identification of the product in question, it is noted that it comes in a white container; blue lid; and marked with the batch’s: N1082701 (EXP: 050328), GESPM240622 (EXP: 071324), GESPM300622 (EXP:071324), GESPM050722 (EXP:071324), 190620230009B (EXP:061928), 190620231573678B (EXP:062828) and 280620231573678BA (EXP:062828).

Consumers should stop consuming the recalled products. Consumers who have purchased the recalled lots of ELV Control Herbal Supplement 90 capsules can return them to the place where they made their purchase to obtain an exchange for another product marketed by the company.
