- comments: obserevd no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)(citation issued)

- comments: observed male food handler preparing and tasting steak on grill he took several handfulls of steak from grill and placed in his mouth while cooking. Instructed manager on proper procedure and eating in a designated area. (priority 7-38-010)(cos)(citation issued)

- comments: observed several open beverage containers on shelving units above prep areas without lids or straws. Must be stored properly to prevent contamination.

- comments: observed hand washing sink not accessible in rear prep area. Observed a garden hose attached to faucet and sitting inside of basin and a spatula as well in same while food is being prepared. - comments: observed several open and closed bottles of toxic chemical such as (peroxide,alcohol,oven degreaser,bleach,oven spray)stored above prep tables inside of food container with open food below ,next to food on lower shelf of grill table,throughout lower shelf of prep table next to all open foods. - comments: observed evidence of appx. 12- 15 live roaches crawling in door gaskets of 3- door reach in cooler in rear storage area,on door and door jams of swinging door leading to same area,on floor behind large food containers and under ice machine and several dead roaches on floor throughout rear storage area. Also. Appx. 5-7 live small flies on walls in ice machine/food storage area. Recommended to have a pest control operator to service premises on or after 1/6/22. Observed a female employee walk in around without a face covering and male food handler with mask underneath his chin several times during inspection.
