- comments: no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken,coleslaw,french fries,etc) must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: observed no handwashing signage at handsinks near 3 compartment sink and in toilet room. Instructed manager to provide and maintain.

- comments: observed cooked chicken stored at improper temperature in no temperature controlled unit ranging from 78. 0f - 84. 2f. Manager discarded and denatured approximately 15lbs and $239. All hot tcs food must maintain 135f or above during storage, preparation, and service. Priority violation. Citation issued 7-38-005

- comments: observed raw fish and raw chicken in prep area not stored in a temperature controlled unit. Chicken is at 51. 6f and fish is at 60. 0f. Instructed manager that cold foods must remain at 41. 0f or below. Manager discarded and denatured 20lbs at $100. Priority violation 7-38-005. Citation issued

- comments: observed no probe thermometer for taking food temperatures. Must provide. Priority foundation (7-38-005) citation issued

- comments: observed 1/4' gap on side of back door. Instructed manager to seal to prevent the entrance of unwanted pests.

- comments: observed cardboard being used to line shelves in front prep area. Instructed manager to remove. All surfaces must be easily cleanable.

- comments: observed no sanitizing test strips for 3 compartment sink. Instructed manager to provide to test strips for measuring the correct sanitizing concentration. Priority foundation 7-38-005. Citation issued

- comments: observed hot water fixture at handsink next to 3 compartment sink not functioning properly. Instructed manager to repair and maintan.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in toilet room. Instructed to provide and maintain.
