- comments: observed no handwashing signs at hand washing sinks in rear prep and washroom areas. Instructed manager to provide.

- comments: observed chicken (109. 6f-123. 5f)stored at improper
temperature in hot holding unit. Instructed manager to discard approximately 50lbs and $100 of cooked chicken. All hot tcs food must maintain 135f or above during storage, preparation, display and service. Priority 7-38-005 citation issued

- comments: observed the following cold tcs foods stored at improper temperature in walk-in cooler: salmon 45. 3f, catfish 46. 2f-48. 5f, curry chicken 48. 5f, tilapian 48. 5f, lamb chops 48. 5f, turkey legs 48. 5f, lobster tails 49. 6f. Instructed manager to discard approximately 100lbs and $854 of product. All cold tcs food must maintain 41f or below during storage, preparation, display and service. - comments: observed walk-in cooler not maintaining proper temperature 50. 2f. Instructed manager to repair. Walk-in cooler must maintain 41f or below. Walk-in cooler is tagged held for inspection and must not be used until reinspected by chicago department of public health. Priority 7-38-005 citation issued

- comments: observed bulk containers not labeled in rear prep/storage area. Instructed manager to label all bulk containers with common name of food.

- comments: observed food items stored on floor in front counter(bread and jerk sauce), prep(chicken and fish) and storage areas. Instructed manager to elevate all food items six inches off floor.

- comments: observed single service articles not inverted. Instructed manager to invert all single service articles to protect from contamination.

- comments: observed damaged and missing gaskets on doors of walk-in and reach-in coolers. Instructed manager to replace and maintain.

- comments: observed no chemical test kit available. Instructed manager to provide test kit for proper concentration in sanitizing solutions. Priority foundation 7-38-005 citation issued.

- comments: observed food debris and grease buildup on interior and exterior surfaces of cooking equipment, prep tables, shelving units, coolers, and freezers. Instructed manager to clean and maintain.

- comments: observed leaking drainpipe under 3-compartment sink. Instructed manager to repair and maintain.

- comments: observed no self-closing device on men washroom door. Instructed manager to provide.

- comments: observed dust buildup and soot on ceiling tiles, ceiling vents, and light shields in prep area. Instructed manager to clean and maintain.
