- comments: person in charge did not have a city of chicago sanitation certificate. Priority foundation violation.

- comments: no city of chicago sanitation
certificate posted on the premises or such a person on site while tcs foods were being prepared and served. Priority foundation violation 7-38-012 citation issued.

- comments: found no hand sink installed at rear dishwashing area. Must install a hand washing sink at the dish washing area with hot and cold running water, soap and paper towels. Priority foundation citation issued 7-38-030c.

- comments: observed cooked tcs pizzas held at improper hot holding temperatures. Cheese pizza at 73. 9f, meat pizza at 72. 6f, sausage pizza at 90. 5f. Reviewed all hot foods must be held at 135f or above. No temperature logs on site. Priority violation 7-38-005 citation issued.

- comments: no metal stem thermometer on site to measure food temperatures and monitor. Must provide. Priority foundation violation 7-38-005 citation issued.

- comments: all bulk food containers must be labeled with the product common name.

- comments: noted holes in the mesh screens of both the front and rear exit screened doors being used during the inspection. Must provide tight fitting mesh screens without holes/openings.

- comments: observed 50 to 75 mice droppings throughout the hvac storage closet in the rear food prep area. Instructed to remove all droppings, sanitize all affected areas, and recommend having the pest co. Service for mice. Priority foundation violation 7-38-020(a)

- comments: dirty shelving throughout the rear food prep, rear dry storage, front prep. All with dirt and food debris. Must clean and maintain.

- comments: all clutter and unnecessary items must be removed from the rear hvac storage closet. All need items must be elevated and organized.

Must clean and maintain. -----holes in walls throughout the rear dish and mop sink area. Must seal all holes/openings in walls. -----water damaged ceiling tiles throughout the entire facility. Must replace all. Missing baseboard coving throughout all prep, dish, and washroom. Must provide sealed coving along baseboards and seal all openings along floor seams. ----missing floor drain cover at the three-compartment sink. Must provide. -----must not use duct tape as a sealant for walls at the walk-in cooler. ----missing and damaged floor tiles at the threshold front the front to rear prep areas and on the pizza prep area. Must replace/repair.

- comments: no food handler training for for any employees. Must provide.
62. Compliance with clean indoor air ordinance
- comments: no posting of required signage at the front entrance or the rear entrance to the facility. Must provide required signage. Citation issued 7-32-020(a)
