- comments: observed no employee health policy/training on site. Instructed facility to establish an appropriate employee health policy/training system and maintain with verifiable documents on site. Priority foundation violation 7-38-010. No citation issued

- comments: found procedure/plan on-site; however, kit not complete missing solution to control norovirus for responding to vomiting and diarrheal events, also missing mop etc. Instructed facility to maintain any appropriate supplies on site. - comments: we observed live roaches (15 or more) at rubber gaskets and hinges of the interior of two door reach-in cooler(roaches on both sides of doors) under prep table in the kitchen. On the second floor, we observed over 200 rodent droppings under shelves in the food storage area and behind the bar area. Instructed to remove roaches and droppings, clean and sanitize affected areas. Contact pest control company. Priority violation:7-38-020(a), citation issued
