- comments: observed black mold like substance on interior of ice machine panel dripping on ice stored in bin below. (ice used for human consumption). Instructed person in charge to dispose of ice, clean and sanitize ice machine and maintain. Citation issued. #7-38-005 priority foundation.

- comments: observed bulk containers of beans and rice not labeled with the common name of the ingredient. Instructed the person in charge to label all bulk containers with the common name of the ingredient and maintain.

- comments: observed pipe on ceiling near three compartment sink leaking. Instructed person in charge to repair and maintain.

- comments: observed water-damaged ceiling tiles in the women's restroom ceiling. Instructed person in charge to replace and maintain.
