- comments: observed no valid certified food managers certificate posted to view. Must provide and maintain. (priority foundation 7-38-012)

- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

38. Insects, rodents, & animals not present
- comments: observed an infestation of appx. 2000 or more mice droppings scattered throughout premises such as the kitchen-along wall bases,shelving units,prep tables,hot holding units,storage cabinets,on buckets. Dry food storage area-on every shelving unit on & in between food containers and utensils. Pallets of food stored in hallway & on boxes of food products. Pantry give away area-on floor under,around,along wall bases of shelving units,on top of food boxed products,inside of food storage containers. Dining area-on floor along wall bases underneath high chair and table against wall. Also evidence of a live mouse run across floor in hallway under pallets of food. Also evidence of 2- live roaches crawling on floor under drying counter and inside cabinet on serving line. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 7/31/24. (priority 7-38-020(a)

- comments: observed no pest control log book with required documents must provide and maintain. (priority foundation 7-38-020(c)

- comments: observed a gap along bottom of all exit doors must make tight fitting to prevent rodent entry.

- comments: observed no chemical test kit to check sanitizing solution ppm's at 3- compartment sink. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed hand washing sink in prep area not connected to city plumbing. Observed a plastic drainline hose leading into floor of open grease trap. Must repair and maitain. (priority foundation 7-38-030(c)

52. Sewage & waste water properly disposed
- comments: observed inadequate sewage/waste water disposal. Observed the grease trap in floor under 3- compartment sink with a black hose down inside of sewage/waste water being pumped outside from a opening in the window and hose carried along grass and wall out to the curb into the city sewer system. Must repair and maintain. (priority 7-38-030(c)
