Take out food- a four bowl rice quinoa Parmesan broccoli chicken with avacado on side and citrus avacado dressing. On 10/23/18 for lunch. Sick about 11 hours later vomit and diirrea at same time , terrible. Still feeling ill 630am. Cannot leave my house for work started sick 1am 10/24/18  | Symptoms: Diarrhea, Vomiting

Last Thursday I ate organigirl romaine lettuce and was sick two hours later with diarrhea. I didn’t connect the two issues and had the rest today and am now sick 2.5 hours later with diarrhea. It was purchased at Whole Foods carrollwood  | Symptoms: Diarrhea, Nausea
