- comments: observed no employee health policy on premises as required; must provide. Instructed the pic to provide documentation of employees responsibility to report any illness transmissible through food. Priority foundation violation 7-38-010.

- comments: observed inadequate supplies on site for responding to vomiting and diarrheal events. Must provide. Instructed manager that a vomiting and diarrheal clean up procedure must be documented and the necessary supplies on site in the event of a vomiting and diarrheal incident on premises. Priority foundation violation 7-38-005.

- comments: observed no hand washing sign at rear hand washing sink. Hand washing sign shall be provided at all hand washing sinks used by employees.

- comments: observed bulk containers not labeled in prep area. Instructed manager to label all bulk containers with common name of food.

- comments: observed over 60 mice droppings scattered on floor in southwest corner around water heater in rear prep area, 20 mice droppings scattered on floor in northeast corner near reach-in cooler and electrical panel, 20 mice droppings scattered on floor in corner near 3-compartment sink, grease trap and stand mixer. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all food items six inches off floor.

- comments: observed soiled wiping cloths stored on prep tables. Instructed manager to store in sanitizing solution between uses.

- comments: observed single service articles not inverted in wait station area and single service articles stored on floor in dining area. Instructed manger to invert and elevate all single service articles off floor and maintain.

- comments: observed open food items stored in grocery bags in chest freezer near water heater. Instructed manager to properly store in food bags or containers.

- comments: observed accumulated grease, food debris, dead roaches on interior and exterior surfaces of fryers, fryer cabinets, coolers, freezers, prep tables, holding units, shelving units, mixers. Observed calcium buildup on interior surfaces of ice machine. Instructed manager to clean and maintain.

- comments: observed leaking faucet on 3-compartment sink. Instructed manager to repair and maintain.

- comments: observed no covered waste receptacle in unisex washroom. Instructed manager to provide.
