- comments: no written employee health policy on site. Must provide and maintain for review. Priority foundation violation 7-38-010 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: no written cleaning procedure for a vomiting/diarrhea event and required equipment. Must provide. Priority foundation violation 7-38-005. Citation issued.

15. Food separated and protected
- comments: observed approximately 10 lbs cooked, ready to eat shrimp being stored on display in the raw seafood display cooler in between raw shrimp, directly next to and touching the raw shrimp. Instructed all ready to eat and raw animal origin foods must be stored separated and protected. Shrimp discarded, estimated value $50. 00. Priority violation 7-38-005 citation issued.

20. Proper cooling time and temperature
- comments: observed approximately 10 lbs cooked pork in a deep metal container with a metal lid inside the cooks line two door cooler at an improper temperature of 70f. Management states the pork was cooked that day. No time/temperature logs on site. Reviewed with the manager proper cooling procedures of using loosely covered shallow containers, quick chills in freezers/times required for the cooling process. Pork discarded. Estimated value at $10. 00 priority violation 7-38-005 citation issued.

21. Proper hot holding temperatures
- comments: observed approximately 15 lbs cooked whole chicken, pre-packaged in a hot holding display unit at inadequate temperatures of 123. 1f & 128. 1f. Instructed all hot foods must be held at a hot holding temperature of 135f or above. All discarded. Estimated value $50. 00. Priority violation 7-38-005. See above violation #20 for citation.

25. Consumer advisory provided for raw/undercooked food
- comments: observed a poster wall menu of assorted breakfast egg items that may be cooked to order. Menu does not disclose and inform consumers the specific menu items that are raw or under cooked and a potential hazard of consuming such food. Must provide a consumer advisory that discloses and reminds customers of such items. Priority foundation violation 7-38-005. No citation issued.

- comments: all bulk food containers must be properly labeled with the product common name.

- comments: excessive dust on the north washroom ceiling ventilation. Must clean and maintain. Must not use cardboard as matting for the floors throughout.
