Chicken bowl with brown rice, hot salsa, cheese, and sour cream. Incident occurred on 8/10/2107 at 12:15 pm. A trip to the emergency room for 2 IV bags with an anti-nausea medicine was necessary as I became severely dehydrated from the vomiting and diarrhea.  | Symptoms: Diarrhea, Fever, Nausea, Vomiting

5pm on Aug 11 we ate Five guys and was sick within an hour or so. Severe stomach cramps, nausea, and extreme diarrhea that went on for 2 hours. The only difference in our meals was a chocolate shake.  | Symptoms: Diarrhea, Nausea, Cramps
