- comments: no soap provided at handwashing sink at sushi area. Employee provided it after my request. Priority foundation violation:7-38-030(c),citation issued

23. Proper date marking and disposition
- comments: must provide proper labels for date marking the preparation and consume by/discard date (maximum of 7 days total) of all refrigerated, ready-to-eat, tcs foods prepared on site and held over 24hrs. Priority foundation violation#: 7-38-005. No citation issued. -

38. Insects, rodents, & animals not present
- comments: evidence of live roaches in different area of premises. One baby roach under the three compartment sink, 4 live baby roaches inside the door of reach-in cooler accross cooking equipment,one live adult roach inside the two door reach-in cooler at rear prep area. Instructed to remove roaches,clean,sanitize and maintain mentioned area. Contact pest control. Priority foundation violation:7-38-020(a),citation issued

- comments: splash guard must be installed(not a removable splash guard) in front sushi prep area in handwashing sink adjacen to food prep surface.

- comments: all wiping cloths in all prep areas must be held in a container with a sanitizing solution between use.

- comments: rusty shelves at rear food storage area. Surface must be smooth,cleanable and non-absorbent material

- comments: observed chest freezer near rear exit door with rust on lid and at bottom all surfaces need to be restored(which previousely was done) or replaced to a smooth and easily cleanable surface and maintained.

- comments: no chlorine chemical test strips on site to check sanitizing levels for the three compartment sink and low temp dish machine must provide chlorine test strips. Priority foundation violation 7-38-005 citation issued.

- comments: no back flow prevention device provided at ice machine water line,at prep area must install so back flow prevention device may be located to be serviced and maintain.

- comments: debris on floors along baseboard walls,under and behind shelves and coolers. Clean and maintain

- comments: unknown particles at light shield in prep and dishroom, instructed to clean and maintain

- comments: must provide food handler trainning certificate,for all employees
