- comments: 6-501. 111: observed evidence of pest activity. Observed approximately 45 mice droppings throughout the rear food prep area. The following areas were observed with mice droppings: on the floors near the orange carbon dioxide tanks near the 1 compartment sink in the dishwashing area, under and on the sides of the floors where the metal shelving units and food prep table in the rear hallway near the dishwashing area, in the cabinet under the handwahsing sink in the front food prep area, in the lower cabinets under the register and the front hot and cold holding units behind boxes. Instructed to eliminate evidence of pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation violation 7-38-020(a), citation issued
51. Plumbing installed; proper backflow devices
- comments: 5-205. 15: observed a leak at the wash compartment of the three compartment sink. Instructed to repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 13: observed the floors and the floor drains under the three compartment sink with debris. Instructed to clean and maintain said floors.
58. Allergen training as required
- comments: 2-102. 13: observed manager on site with no allergen training certificate. Instructed all certified food managers must take allergen course and maintain records on site.
