- comments: 2-101. 11 observed the person in charge without a original/valid city of chicago food service sanitation manager's certificate. Person in charge presented a servsafe certificate with the examination date of 3/2023. Instructed all persons in charge must obtain the city of chicago food service sanitation manager's certificate through kennedy king college within 3 months of the serv safe examination date. - comments: 2-401. 11 observed food handlers drinking from cups without lids in the food prep area. Instructed to provide lids for all food handlers drinking cups at all times.

- comments: 4-501. 114 observed the low temperature dish machine not properly sanitizing at 0ppm during the inspection. Instructed to always properly sanitize at 50ppm-100ppm of chlorine at all times. Dish machine technician arrived and repaired said dish machine and by the end of the inspection said dish machine was maintaining 100ppm. Priority violation. Citation issued 7-38-025.

- comments: 4-602. 11 observed stained cutting boards. Instructed to remove stains or replace the cutting boards. Observed minor accumulated debris inside of the soda gun holder behind the bar. Instructed to clean and sanitize.

- comments: 3-603. 11 observed no consumer advisory disclosure on the digital menu for all items that can be ordered undercooked or raw such as steak frites, all poached egg items on the brunch menu and the biscuits and gravy with sunny side up eggs item. Instructed to add a disclosure on the digital menu for all items that are offered undercooked or raw. Priority foundation 7-38-005.

- comments: 6-501. 111 observed evidence of live insect activity on site. Observed about 10 live small flies and about 5 live large flies flying around in the basement and about 5 live small flies flying around the bar area and about 20 live small flies in the toilet room located closer to the bar. Instructed to eliminate live insect activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 3-303. 11 observed ice used for human consumption in a bin with glass wine bottles submerged in the ice. Instructed to remove the wine bottles, discard the ice and never contaminate ice that is used for human consumption. Manager removed wine bottles and discarded ice during the inspection. Priority violation. Citation issued 7-38-005.
