- comments: observed the establishment serving under cooked meat (burgers) with no consumer advisory, disclosure or reminders to the customers. Instructed to have a consumer advisory on the menu with a disclosure notice and reminder statement. Also must indicate on menu 'cook to order' phrase for burgers. - comments: rusty door liner at inside walk-in cooler,also prep table base is rusty unable to clean. Surface must be smooth cleanable and non-absorbent surface.

Instructed to clean, sanitize and maintain unit. Ice is used for human consumption. Priority foundation violation:7-38-005 presently we still observed black slime substance at interior of unit,specially around screws and knobs. Must detail and sanitize unit priority violation:7-42-090
