- comments: observed no certified food manager on duty while tcs ready to eat foods are being prepared,handled and served such as (italian beef,ground beef,cut fruit,sandwiches,etc)must be on site at all times. (priority foundation 7-38-012)

- comments: observed evidence of appx. 40 or more mice droppings in both lower cabinets at coffee station and a dead mouse inside of coffee box in same. Must remove droppings and dead mouse. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/20/20. (priority foundation 7-38-020(a)

- comments: observed an appx. '1/2-3/4' gap along bottom of rear door. Must make door tight fitting.

- comments: must remove rust from grease trap at 3- compartment sink and rusty shelving units in walk in cooler.
